Complete care, coordinated in one platform.
Five integrated service categories, from first assessment to laboratory follow-up, all accessible through the Apomuden platform.
Integrated Clinical Services
Five coordinated service categories, from first assessment to laboratory follow-up, all accessible through a single platform, documented in one shared health record.
Every service is priced transparently, delivered by verified providers, and recorded in the patient's permanent digital health record.

Home-Based Care
Essential clinical services delivered directly to the patient's home by a verified registered nurse. Includes vital signs assessment, blood pressure monitoring, blood glucose testing, wound care, and basic diagnostics. All visits are conducted using DigiCare-issued standardised equipment and documented in the patient's digital health record on completion.
Doctor Access
Where a patient requires a physician assessment, a verified General Practitioner is available for a home consultation through the Apomuden platform. The doctor conducts a full clinical review, documents findings, and initiates referral or escalation where clinically indicated. Doctor home visits through Apomuden are priced below the equivalent cost of a private facility consultation.
Pharmacy Coordination
Where medicines are recommended during a home visit, patients can request fulfilment directly through the Apomuden app. The platform coordinates the order with the nearest verified partner pharmacy. Partner pharmacies operate under a formal platform agreement and provide delivery where capacity allows.
Laboratory Services
Where diagnostic tests are required, the platform coordinates with verified partner laboratories to arrange home sample collection or guided facility visits. Test results are returned digitally through the platform. Abnormal or critical values are automatically flagged to the clinical team, ensuring findings are reviewed without relying on the patient to act alone.
Preventive Care & Chronic Condition Monitoring
Apomuden is designed specifically for the ongoing monitoring and early detection of the chronic conditions most prevalent in Ghana: hypertension and diabetes. Regular structured visits build a clinical picture over time, identify deteriorating readings before they become crises, and create the continuity of care that informal and ad hoc health interactions cannot provide.
What This Enables
A coordinated healthcare experience where patients receive continuous, timely, and connected care across all service points. It reduces fragmentation between providers, improves speed of response, ensures accurate follow-up, and maintains a complete digital health record that supports better clinical decisions and long-term health outcomes within the community.
Pricing & Transparency
All service fees are clearly displayed to patients before booking confirmation. Transport and service charges are shown separately with a transparent total, ensuring full clarity on costs prior to service delivery. DigiCare is designed around a transparent, user-friendly pricing structure that supports both direct payments and evolving healthcare financing models. Where applicable, services that may align with public or private insurance frameworks are indicated within the platform to support future integration pathways. The system is being developed to remain compatible with national health financing structures as they evolve, enabling potential future collaboration with insurance providers and public health schemes for expanded access to care.
